What closes the loop

  • The left half, signal to mitigate, is what the paging path and response tooling do tonight. The right half, review to hygiene, is what makes next quarter's nights quieter.
  • Each stage produces an event the next one consumes: the review is generated from the timeline, action items from the review, the noise report from the ledger and the incident history. No stage depends on someone remembering to copy something.

Measures

  • Reviews completed on time, action items closed with a linked change, recurring contributing factors, actionable page rate and out-of-hours interrupts. A loop that is closing shows the last two improving.

Failure of the loop

  • A platform with excellent paging and no learning loop pages the same people for the same causes forever. It looks reliable on its own dashboards while the rotation burns out.
